In the Engine room, while 3rd Engineer was shifting one spare main engine exhaust valve by crane from aft to forward parking place, the chain of the crane got entangled with no:2 unit fuel oil feed pipe and the valve cracked, spilling HO with 5 bar pressure at 120degree C all over the engine room port side.

Engine was immediately shut down and booster P/p shut off.

There was no fire due to this, but lots of oil spilled in the engine room which was then cleaned up.

This was due to oversight of the 3rd Engineer while operating the engine room crane. He was advised caution when using the crane in future.
