At sea after sailing out from Korean port, heavy knocking sound was heard from camshaft of M.E. This knocking sound was not continuous; in between it stopped.

Through cam shaft inspection cover noticed one unit fuel injection pump roller and guide was getting stuck, and again after some revolutions would move down with impact.

Injection pumps and roller guide were removed. Observed roller and pin damaged and guide also had some deep scratches.

Most proximate cause:- Roller pin lubrication hole alignment displaced as such type has no provision to keep pin locked in particular aligned positions. On displacement of pin, lubrication of pin & roller stopped and caused the damage.

New roller/ pin/ guide fitted again without any locking screws and marked. At first port with help of shore workshop arrangement fitted back.
