Safety of crew members onboard is the "responsibility of any one who carries out the job, who supervises the job, and who authorizes the job.

When any one in the system fails to assume responsibility, there is a danger to the personnel on board.

A Third officer perished in an incident as he failed to observe the above principle when working on deck.

He was crushed between two containers on deck that were being loaded. Hence the training on board and various modular courses which are aimed at cultivating the above principle.
