The vessel was at an anchorage. The fifth engineer wanted to transfer water from the E/R bilges to the bilge holding tank.

When switched on after setting the lines the pump didn't rotate. For the sake of checking the belt tension, he put his hand on the belt. But then the pump suddenly started rotating, entrapping his fingers in the pulley groove. All fingers were severed.

The lessons learnt from the incident are:

1) The pump belt should not have been slack
2) Before doing any work on machinery, the machine should be switched off.
