IMPROPER SUPERVISION AND INSTRUCTIONS LEADING TO A NEAR MISS INCIDENT

After completion of unloading of palm oils, tank cleaning was undertaken. The next cargo to be loaded was ‘unleaded gasolene’. Tank cleaning was done as and when the parcels were discharged. 2P tank was cleaned and kept ready for loading gasoline. Chief Officer, instructed the pump man to blank the steam line for 2P tank. Chief Officer took it for granted that the pump man is aware of the safety procedures. Pump man being of another nationality, thought that chief officer has checked everything and then only he has given the work to him, he started openting the steam line without checking whether the master valve to tank heating line is shut. The steam started gushing out, luckily he escaped injuries by getting away from the place.
Lesson learnt :
Whether the person is aware or not, each and everytime the job is being given, it is important for the senior person to check whether the operator is aware about the situation and he can do the job safely.
