On one of the vessels manned by our company, the deck crew along with an experienced pumpman went on deck to lash lube oil drums. But as the weather became worse the crew left the job half-way through and came back to a safer place near the accommodation.

The pumpman however perhaps from over-confidence wanted to complete the job and so carried on nevertheless. But as fortune would have it, a big wave washed aboard and carried him away.

Despite a long search we were unable to find his body in the inclement weather.

1. Drums should have been well secured prior to the onset of inclement weather.
2. Those sent out on deck should have been using safety harnesses and lines.
3. Ship's course and speed should have been adjusted to ensure no seas would be shipped while the men were on deck.
