One of the UMS class vessels of my company lost an auxiliary engine crank shaft due to lack of proper decision at the right time.

UMS alarm attended by a junior engineer found L.O. leakage from the A/E engine. At the same time a low pressure alarm also sounded. Since the D/E did not stop the A/E on low pressure nor stop the genie manually, all the bearings together with the crank shaft were badly damaged .

If the A/E had been stopped at the right instant this incident could have been averted.
