DAMAGED THREADS CAUSE ACCIDENT

We were lapping the main engine exhaust-valve seat with spindle.
The spindle was threaded to an eyebolt. After the lapping was finished, we lifted up the spindle and were cleaning the face, when suddenly the eyebolt came loose at the threads and crashed down.
The 3rd Engineer who was cleaning the face at the time had his finger caught in between which got badly cut. Being in port he was sent to the doctor and he got three stitches on his finger.
Later we found that the threads on the spindle were damaged, because of which the eye bolt had come loose.
