Auxiliary Engine Fuel-pump side had a protective cover which was removed because 4th engineer was working on fuel valves and pumps on all 4 engines.

One day the high-pressure pipe from fuel pump leaked, spraying fuel on the exhaust piping of another running engine.

A fire started up and started spreading.

Luckily even though the 5th engineer in the night shift was new and it happened during the early morning at 0200 hrs, the immedediate use of a fire extinguisher brought the fire under instant control. The alarm brought the other men out and the fire was put out successfully.

All this happened in the first place because the protective cover on the fuel pump side was not fitted back, due to sheer negligence.
