The accident took place in one of the reefer container vessels in Newzeland. The container was being loaded in 2nd tier, and the 2nd mate, being duty officers, was holding the container on sides for support and was checking up the photo log chart for the set temperature and the actual plugged in temperature. At the same time another container was lowered by the side of it, which he did not observe.His fingers got jammed between the two containers and he lost all the 5 fingers, a permanent loss for his rest of life.

Lessons Learnt :

always be careful while attending to cargo work. Do not hurry up the work and at the same time do not delay too much. You have to be aware of what is happening surrounding you and exercise all safety measures.
