PIECE OF RAG IN THE LUBRICATING OIL FILTER CAUSING THE DAMAGE TO THE BEARING
The ship sailed from Port Blair. After dropping the pilot, when the vessel was passing the channel, there was a crankcase explosion. Engine stopped and on investigating, found the crankcase relief door of unit 6 had opened. Engine was allowed to cool, and the crankcase doors were opened up. Found the unit 6 bottom end bearing had seized onto the pin. On further investigation, it was found that the lubricating oil line got blocked with a piece of jute/rag which had obstructed the lub oil passage to the bearing.
In the port, the junior engineer and the oilman had cleaned the filter and they boxed back the same. No senior engineer supervised or inspected the job before boxing up.Lesson learn t: Lubricating oil filters and fuel oil filters, whenever being cleaned, it has to be supervised by seniors before boxing back.
