IMPROPER CHECKS AND MAINTENANCE LEADING TO CHAOS
During a routine drill, the life boat was boarded and all pre-launch procedures adhered to.
The boat was a free fall boat. Engine was started and the hatch was secured. The boat was released and was water borne, due to a leak in the exhaust of the engine, the people inside felt uncomfortable. The cabin pressure started slowly increasing and hence the doors did not open. Panic situation prevailed, and after sometime, it was managed to open the doors with the help of fire axe. After returning and ‘hauling in’ the boat, checks were made. The safety valve which should have opened when the cabin pressure increased was found stuck.
Lessons learnt :
during the weekly drill, all safety checks to be made and atleast all the leaks can be identified and rectified. Safety alarms and valves to check religiously and honestly.
